LOFSA CONSENT FORM

As per State Law, parents with students in 8th - 12th grade are required to make an annual election either consenting to or denying the student's high school to collect and share the student's Personally Identifiable Information (PII) and disclose it to the Louisiana Office of Student Financial Assistance (LOSFA) for TOPS and other financial aid or to the state’s colleges and universities for admissions.  Student transcript data will not be provided to LOSFA or college institutions without parental permission. Please read the Consent Form, fill out and sign the portion of the form that applies to your decision to grant or deny consent, and return the form to the school.
